ABSTRACT

El neumoencéfalo es una patología que comúnmente se presenta después de cirugía neuroquirúrgica y ocasionalmente endonasal. Estos se suelen manejar de manera conservadora, sin embargo, se pueden asociar a distintas etiologías las cuales los hacen recurrir. En este reporte presentamos dos casos de neumoencéfalo tardío post quirúrgico asociado a fístulas de LCR de bajo flujo, donde se discute su clínica, etiología y manejo posterior.


Pneumocephalus is a pathology that commonly occurs after endonasal surgery, these are usually managed conservatively, however they can be associated with different etiologies which make them recur. In this report we present two cases of post-surgical late pneumocephalus associated with low-flow CSF fistulae, where its symptoms, etiology, and subsequent management are discussed.

Resumen Objetivo: El objetivo de este manuscrito es presentar el caso de un varón de 41 años que debuta con shock séptico y fascitis necrotizante abdominal en el posoperatorio del desbridamiento de un absceso perianal para focalizar la atención del lector en la posible evolución clínica hacia gangrena de Fournier. Materiales y Método: Tras la intervención, el paciente refiere aumento de temperatura y sensación de crepitación subcutánea a nivel abdominal, junto con empeoramiento clínico y hemodinámico, evidenciándose evolución tórpida hacia gangrena de Fournier extendida a región abdominal. Resultados: Tras la reintervención, el paciente presentó una evolución favorable aunque requirió sucesivas curas y desbridamientos quirúrgicos. Conclusiones y Discusión: Cabe destacar la importancia de una exploración clínica completa y detallada previa a cualquier intervención quirúrgica, así como el diagnóstico temprano en situaciones de shock séptico que permitan inicio de antibioterapia precoz y control del foco eficaz.


Aim: The objective of this manuscript is to present the case of a 41-year-old man with septic shock and abdominal necrotizing fasciitis after drainage of an interesphinteric perianal abscess to focus the reader's attention on the possible clinical evolution towards Fournier's gangrene. Materials and Method: After the intervention, the patient reported an increase in temperature and a sensation of subcutaneous crepitus at the abdominal level, with clinical and hemodynamic worsening, showing a torpid evolution towards Fournier's gangrene extended to the abdominal area. Results: After the reoperation, the patient presented a favorable evolution, although he required successive cures and surgical debridements. Conclusions and Discussion: It is worth highlighting the importance of a complete and detailed clinical examination prior to any surgical intervention, as well as the early diagnosis in situations of septic shock that allow early initiation of antibiotic therapy and effective control of the focus.

ABSTRACT

La sacroileítis infecciosa (SII), también descrita en la literatura como sacroileítis séptica o piógena, es una patología infrecuente, y su diagnóstico constituye un reto debido a su rareza relativa y la diversa presentación clínica, que frecuentemente imita otros trastornos más prevalentes originados en estructuras vecinas. Se requiere un alto índice de sospecha y un examen físico acucioso para un diagnóstico oportuno, mientras que los estudios de laboratorio y de imagen ayudan a confirmar el diagnóstico y dirigir la estrategia de tratamiento apropiada para evitar complicaciones y secuelas a corto y mediano plazos. Presentamos un caso de paciente de género femenino de 36 años, con cuadro clínico de SII izquierda, secundaria a un absceso del músculo iliopsoas, condición que generalmente se presenta como una complicación de la infección. Se realizaron los diagnósticos clínico, imagenológico y Biológico, se inició el tratamiento antibiótico oportuno, y se logró una excelente evolución clínica, sin secuelas


Infectious sacroiliitis (ISI), also described in the literature as septic or pyogenic sacroiliitis, is an infrequent pathology, and its diagnosis constitutes a challenge due to its relative rarity and the diverse clinical presentation, frequently imitating other more prevalent disorders originating in neighboring structures. A high index of suspicion and a thorough physical examination are required in order to establish an opportune diagnosis, while laboratory and imaging studies help confirm the diagnosis and direct the appropriate treatment strategy to avoid complications and sequelae in the short and medium terms. We herein present a case of a female patient aged 36 years, with a clinical picture of left ISI, secondary to an iliopsoas muscle abscess, a condition that usually presents as a complication of the infection. The clinical, imaging and microbiological diagnoses were made, the timely antibiotic treatment was initiated, and an excellent clinical evolution without sequelae was achieved.

Resumen Introducción: En un quiste hidatídico hepático pueden ocurrir una serie de complicaciones de diversa gravedad. Una es el tránsito hepatotorácico (THT), que es el compromiso simultáneo de hígado, diafragma y pulmón secundario a migración de un quiste hidatídico hepático. Objetivo: Presentar una complicación de baja incidencia de un quiste hidatídico hepático como lo es el THT con fistula biliobronquial y posterior resolución quirúrgica. Materiales y Método: Registro clínico, imagenológico y fotográfico del episodio clínico. Resultados: Paciente con diagnóstico de quiste hidatídico en tránsito hepatotorácico con fístula biliobronquial fue sometida a toracotomía, resección en cuña del pulmón incluyendo bronquios comunicantes con el quiste. Paciente presenta evolución clínica e imagenológica favorable. Discusión: Se discuten formas de presentación, complicaciones de la evolución, grados de progresión y ubicaciones anatómicas frecuentes. Se hace énfasis en rol de la clínica e imagenología para diagnóstico y lo controversial del manejo. Conclusión: Tratamiento quirúrgico dependerá de localización de la lesión, estado del quiste, tamaño y experiencia del equipo quirúrgico, siendo una quistectomía con tratamiento de los trayectos fistulosos una buena alternativa.


Introduction: A variety of severe complications can occur in a hepatic hydatid cyst. One of them is the transit from liver to thorax through the diaphragm (HTT). Aim: To present a low impact complication of a hepatic hydatid cyst such as HTT with bronchobiliary fistula and subsequent surgical procedure. Materials and Method: Clinical, imaging, and photographic record of the clinical event. Results: A patient with a diagnosis of hydatid cyst in hepatothoracic transit with bronchobiliary fistula underwent thoracotomy with a wedge resection of the lung, including a cyst-bronchial communication. The patient presents good clinical and imaging evolution. Discussion: Forms of presentation, evolutionary complications, stages of progression, and frequent anatomical locations are discussed. Emphasis is made on the role of the clinical examination and imaging tests for diagnosis and controversial management. Conclusión: Surgical treatment will depend on location of the lesion, experience of the surgical team, condition and size of the cyst, being a cystectomy with the treatment of the fistulous tracts a good alternative.

ABSTRACT

El pseudoaneurisma se define como un hematoma pulsátil repermeabilizado, encapsulado y en comunicación con la luz de un vaso dañado. Se origina cuando hay una disrupción de la pared arterial. La hemoptisis es un signo/síntoma de presentación muy rara de aneurisma aórtico torácico y de pseudoaneurisma aórtico torácico. Hay poca información sobre la hemoptisis asociada con la ruptura del aneurisma aórtico cuyo mecanismo no se explica por la presencia de una fístula aortopulmonar. Entre las hipótesis para explicar este fenómeno, se encuentra la capacidad de las arterias bronquiales de volverse hiperplásicas y tortuosas en presencia de una lesión que modifica la arquitectura pulmonar, siendo más susceptibles a la ruptura. También hay descripciones de lesiones directas del parénquima pulmonar por aneurisma roto. El presente caso nos ilustra que debemos considerar a la hemoptisis como signo de alarma en el diagnóstico diferencial de los aneurismas y pseudoaneurismas aórticos entre otras causas que puede ser fatal en breve tiempo por una hemorragia masiva.


Pseudoaneurysm is defined as a reperfused pulsatile hematoma, encapsulated and communicated with the damaged vessel's lumen. It originates when there is a disruption of the arterial wall. Hemoptysis is a very rare sign/symptom of a thoracic aortic aneurysm or pseudoaneurysm. There is little information on hemoptysis associated with aortic aneurysm rupture, whose mechanisms are not explained by the presence of an aortopulmonary fistula. Among the hypotheses to explain this phenomenon, is the ability of the bronchial arteries to become hyperplasic and tortuous in the presence of a lesion that modifies the pulmonary architecture, being more susceptible to rupture. There are also descriptions of direct lung parenchymal injury from ruptured aneurysm. The present case illustrates that we must consider the hemoptysis as a warning sign in differential diagnosis of aortic aneurysms and pseudo aneurysms, among other causes, that it can be fatal in a short time due to massive hemorrhage.

ABSTRACT

We present a case of organized chronic pneumonia (OCP), a rare interstitial disease, which usually affects people over 50 years of age and is very unusual in pediatrics, so we thought it was of interest to communicate it. Is a 14-year-old male in whom in the study of a febrile condition, a pneumonia of LII was detected. The images persist after 2 months of being treated and being asymptomatic. Infectious causes and rheumatic diseases were ruled out due to the history of discoid lupus. Chest CT, bronchoalveolar lavage and lung biopsy showing Masson's bodies were performed and OCP was diagnosed. He received prednisone 1 mg / kg day and Clarithromycin for 2 months with rapid improvement. Relapses with new radiological images typical of OCP twice, when treating on alternate days. It is indicated prednisone 30 mgr / day for 3 months and gradual reduction. At 18 months, being with prednisone 5 mgr day, has a relapse of rapid progression that was treated with pulses of methylprednisolone. The collagen study shows positive lupus anticoagulant and ANA and Systemic Lupus is diagnosed. The bad evolution would be explained because it was a secondary OCP. Mycophenolate associated with prednisone was indicates, which has been used in the OCP and in Lupus. This clinical case shows the importance of radiological follow-up of patients with OCP and the search for secondary OCP causes due to the poor response to corticosteroids.


Se presenta un caso de neumonía crónica organizada (NCO), enfermedad intersticial rara, que afecta habitualmente a mayores de 50 años y muy inusual en pediatría, por lo que creímos de interés comunicarlo. Se trata de un varón de 14 años en el que en el estudio de un cuadro febril se detecta una neumonía de LII cuyas imágenes persisten luego de 2 meses de haber sido tratado y estando asintomático. Se descartaron causas infecciosas y enfermedades reumatológicas por el antecedente de lupus discoide. Se realizó TC de tórax, lavado broncoalveolar y biopsia pulmonar que mostró cuerpos de Masson, con lo que se diagnosticó NCO. Recibió prednisona 1 mgr/ kg día y Claritromicina por 2 meses con una rápida mejoría. Recae con nuevas imágenes radiológicas típicas de NCO dos veces, al pasar de tratamiento diario a días alternos. Se indica 30 mgr/día por 3 meses de prednisona y reducción gradual posterior. A los 18 meses de tratamiento, estando con prednisona 5 mgr día, tiene una recaída de rápida progresión que se trató con pulsos de metilprednisolona. El estudio de colagenosis muestra anticoagulante lúpico y ANA positivos y se diagnóstica Lupus Sistémico. La mala evolución se explicaría porque se trató de una NCO secundaria, ante lo cual indicó micofenolato asociado a prednisona, que se ha usado en la NCO y en el Lupus. Este caso clínico muestra la importancia del seguimiento radiológico de los pacientes con NCO y de la búsqueda de causas secundarias de NCO ante la mala respuesta a corticoides

ABSTRACT

BACKGROUND@#Human infections with zoonotic coronaviruses (CoVs), including severe acute respiratory syndrome (SARS)-CoV and Middle East respiratory syndrome (MERS)-CoV, have raised great public health concern globally. Here, we report a novel bat-origin CoV causing severe and fatal pneumonia in humans.@*METHODS@#We collected clinical data and bronchoalveolar lavage (BAL) specimens from five patients with severe pneumonia from Wuhan Jinyintan Hospital, Hubei province, China. Nucleic acids of the BAL were extracted and subjected to next-generation sequencing. Virus isolation was carried out, and maximum-likelihood phylogenetic trees were constructed.@*RESULTS@#Five patients hospitalized from December 18 to December 29, 2019 presented with fever, cough, and dyspnea accompanied by complications of acute respiratory distress syndrome. Chest radiography revealed diffuse opacities and consolidation. One of these patients died. Sequence results revealed the presence of a previously unknown β-CoV strain in all five patients, with 99.8% to 99.9% nucleotide identities among the isolates. These isolates showed 79.0% nucleotide identity with the sequence of SARS-CoV (GenBank NC_004718) and 51.8% identity with the sequence of MERS-CoV (GenBank NC_019843). The virus is phylogenetically closest to a bat SARS-like CoV (SL-ZC45, GenBank MG772933) with 87.6% to 87.7% nucleotide identity, but is in a separate clade. Moreover, these viruses have a single intact open reading frame gene 8, as a further indicator of bat-origin CoVs. However, the amino acid sequence of the tentative receptor-binding domain resembles that of SARS-CoV, indicating that these viruses might use the same receptor.@*CONCLUSION@#A novel bat-borne CoV was identified that is associated with severe and fatal respiratory disease in humans.

ABSTRACT

La membrana de oxigenación extracorpórea arteriovenosa se utiliza cuando ocurre falla hemodinámica y respiratoria refractaria al tratamiento convencional; brinda soporte hemodinamico transitorio, oxigenacion y remocion de CO2, y permite reposo pulmonar. Asimismo ofrece la posibilidad de realizar ventilación ultraprotectiva y evitar la generación de VILI (Ventilation-Induced Lung Injury). No ha sido demostrada su utilización como una terapia frecuente en los casos con insuficiencia hemodinámica secundaria a shock obstructivo por afectación mediastínica. Presentamos el caso de un paciente con shock obstructivo por una masa mediastínica de origen linfoproliferativo tratada con membrana de circulación arteriovenosa extracorpórea.


The arteriovenous extracorporeal membrane is used in patients with hemodynamic and respiratory failure, unresponsive to conventional treatment. It provides transitory hemodynamic support, oxygenation and removal of CO2, allowing pulmonary rest. Moreover it offers the possibility of ultraprotective ventilation and avoids generation of VILI (Ventilation-Induced Lung Injury). It is not frequently used in patients with hemodynamic failure secondary to obstructive shock due to mediastinal compromise. We present the case of a patient with obstructive shock, mediastinal mass of lymphoproliferative origin that was treated with extracorporeal arteriovenous circulation membrane.

ABSTRACT

Abstract: Gastrointestinal stromal tumor is rare digestive tract mesenchymal tumor, most often in the wall of the stomach. It is a benign neoplasm, but it can become malignant if not treated. We report a case of gastrointestinal stromal tumor that was discovered after abdominal ultrasonography during staging of a patient with primary cutaneous amelanotic melanoma. Mutation in the tyrosine kinase receptor could explain the development of two types of tumors in the same patient.

ABSTRACT

El opérculo torácico se encuentra localizado entre los límites anatómicos del cuello y el tórax. En esta área se puede encontrar diversidad de patologías, entre ellas, de tipo vascular, del tracto digestivo y la vía aérea, de origen linfático y neural. Las lesiones tumorales de tipo benigno o maligno, que se originan en el cuello o en el tórax, pueden diseminarse a través del opérculo; las más comunes son el bocio de origen tiroideo y las neoplasias de origen linfoproliferativo. Es importante que el radiólogo conozca la anatomía y las patologías que se pueden encontrar en el opérculo torácico, pues, por su localización, muchas veces pasan inadvertidas.


The thoracic inlet is located in the anatomic boundaries between the neck and the thorax. There is a wide variety of entities that can be found in this area including lesions of vascular origin, pathology involving the digestive tract and airways, as well as entities affecting the lymphatics and neural structures. Benign and malignant tumors of the thorax and neck can spread through the thoracic inlet, with goiter and lymphoproliferative neoplasms being the most common. It is important for the radiologist to know the anatomy and pathology that can be found in the thoracic inlet as, given its location, it can often been unnoticed.

Introducción: Las hernias internas se definen como una protrusión anormal de una víscera de un compartimento abdominal a otro. Su incidencia es baja <1 % y son la tercera causa más común de obstrucción del intestino delgado en los países desarrollados. Metodología: Se presentan tres casos clínicos de hernias internas que fueron diagnosticados en el servicio de imágenes diagnósticas de la FOSCAL; con una breve revisión de literatura de cada uno y de hernias internas en general. Objetivo: Educar al médico sobre los hallazgos radiológicos y distinguir las características clínicas de estos raros pero importantes tipos de hernias abdominales. Discusión: Las hernias internas son una causa rara de obstrucción intestinal, su diagnóstico preoperatorio es difícil tanto para el clínico como para el radiólogo, por este motivo el riesgo de estrangulación y la tasa de mortalidad continúan siendo altas. A pesar de los diversos métodos de diagnóstico, la tomografía axial computarizada (TAC) ha demostrado ser el mejor método para detectar hernias internas. Como se evidencia en este artículo, los tres diagnósticos fueron hechos por tomografía axial computarizada. Cuando es posible, la tomografía urgente es el método de diagnóstico más útil, que demuestra la localización de los defectos y/o signos de anomalías congénitas poco frecuentes. Conclusión: Las hernias internas son una condición importante pero infradiagnosticada, existen muchos tipos y se presenta de formas clínicas variables; el método diagnóstico más útil es la TAC, pero siempre en primera instancia debe existir la sospecha clínica por parte del médico.


Introduction: Internal hernias are defined as an abnormal protrusion of an abdominal organ through a normal or abnormal mesenteric or peritoneal aperture. with a relatively low incidence (<1%). It is the third most common cause of small bowel obstruction in developed countries. Methodology: We describe 3 cases of internal hernia that were diagnosed in the radiology service at FOSCAL and made a brief review of the literature. Objective: The objective of this article is to educate the physician about the radiological findings and to distinguish the clinical characteristics of these rare but important types of internal abdominal hernias. Discussion: Internal hernias are a rare cause of bowel obstruction, its preoperative diagnosis is difficult for both clinician and radiologist, because the risk of strangulation and the mortality rate remain high. Despite the various diagnostic methods, CT scanning has proven to be the best method to detect internal hernias. As evidenced in this article, the 3 diagnoses of internal hernias were made by computed topography. When possible, urgent tomography is the most useful diagnostic method, which demonstrates the location of defects and / or signs of rare congenital anomalies. Conclusion: Internal hernia is an important and under-diagnosed condition. There are many types and it presents with multiple symptoms. The most useful diagnostic method for its diagnoses is the computed tomography, but there must always be, in the first instance, clinical suspicion from the doctor.

ABSTRACT

As alterações alveolares após a exodontia de um elemento dental desafiam a Implantodontia na busca por técnicas e materiais que devolvam ao rebordo alveolar os tecidos perdidos com previsibilidade e menor morbidade ao paciente. Este relato de casos clínicos mostra a instalação de implantes imediato ou tardio, associados ou não ao procedimento de carga imediata utilizando o enxerto misto da tuberosidade maxilar como alternativa viável para essas reconstruções, tendo como objetivo principal a redução do número de procedimentos cirúrgicos e a previsibilidade no período pós operatório. O osso da tuberosidade maxilar é composto por uma fina camada de osso cortical externo e medular interno que apresentam alta capacidade de revascularização e integração à área receptora. A vantagem em se manter o periósteo entre o enxerto corticomedular do túber e o tecido conjuntivo é conter a reabsorção óssea periosteal na região cervical e melhorar a qualidade do tecido mole. A limitação do uso da tuberosidade maxilar está relacionada a sua dimensão, que pode permitir a reconstrução de um ou dois dentes por área doadora ou, em alguns casos, pode não haver área doadora disponível. Alveolar alterations after extraction constitute a challenge and lead to the search for techniques and materials to reconstruct the lost tissues with previsibility and less morbidity to the patients. These case report shows the placement of immediate implants, delayed implants, with or without the immediate loading procedure and maxillary sinus lift with immediate implant placement using the maxillary tuberosity mixed graft as a viable alternative to these reconstructions aiming to reduce the number of surgical procedures and improve predictability in the post-operative period. The tuberosity jaw bone consists of a thin outer layer of cortical bone and inner medullary bone that have a high capacity of revascularization and integrating into the receiving area. The advantage of keeping the periosteum between tuberosity corticomedular graft and the connective tissue is to contain the periosteal bone resorption in the cervical area and improve the quality of the soft tissue. Limitation to the use of maxillary tuberosity is related to their dimensions that may allow the reconstruction of one or two teeth per donor area or, in some cases, it may not be available. The unique characteristics of the maxillary tuberosity to provide corticomedular bone and subepitelial connective tissue allow their association in the maxillary tuberosity mixed graft and show great versatility for the reconstruction of hard and soft tissue, allowing refined techniques to solve complex cases in a single surgical procedure...

ABSTRACT

Hydatid cyst, a common disease in the world, is usually transmitted to humans through dog feces. Hydatid cyst is caused by Echinococcus granulosus. Diagnostic interventions for hydatid cyst include physical examination and chest x-ray tomography. Although the treatment options of hydatid cyst vary according to the clinical findings of the patients, the primary treatment may be considered as surgery. We herein reported the case of a child hospitalized due to pneumonia who developed anaphylaxis as a result of the rupture of a pulmonary hydatid cyst.

ABSTRACT

Amiloidose cardíaca é causada por um depósito amiloide entre as fibras cardíacas que pode levar a distúrbio de condução, cardiomiopatia restritiva, baixo débito e comprometimento atrial isolado. Relata-se caso de paciente feminina, 66 anos que há três anos iniciou com perda de peso significativa (40,4 kg)evoluindo com edema de membros inferiores. Na investigação observou-se aumento cardíaco global, e o ecocardiograma evidenciou cardiopatia restritiva infiltrativa e derrame pericárdico. Pela alta suspeição clínica, solicitou-se ressonância magnética cardíaca que foi altamente sugestiva de amiloidose cardíaca.O diagnóstico definitivo se confirma pela biopsia endomiocárdica, porém a ressonância cardíaca é descrita como bom método diagnóstico com raros resultados falso-positivos.


Cardiac amyloidosis is caused by amyloid deposits in cardiac fibers that can lead to conduction disturbance, restrictive cardiomyopathy, low output and isolated atrial involvement. This case report described a female patient, 66 years old, with significant weight loss (40.4kg) that began three year sago, progressing to lower limb edema. On investigation, overall cardiac enlargement was noted,with the echocardiogram showing infiltrative restrictive cardiomyopathy and pericardial effusion. High clinical suspicions prompted a request for a cardiac MRI that strongly suggested cardiacamyloidosis. Although definitive diagnoses require endomyocardial biopsies, cardiac MRI is described as a good diagnostic method with rare false-positive results.

ABSTRACT

O osso humano é um tecido de alta mutabilidade. Quando a reabsorção ósseo-alveolar apresenta um nível elevado, a reabilitação oral é prejudicada, principalmente no caso das reabilitações por implantes osseointegrados. Nesse sentido, os enxertos ósseos em bloco passaram a ser utilizados na reconstrução óssea, podendo esses ser autógenos, alógenos, xenógenos ou aloplásticos. Apesar de osso autógeno ser considerado padrão-ouro, uma série de contratempos, como presença de segundo leito cirúrgico e maior dor pós-operatória, levou a busca de uma alternativa nos enxertos alógenos. O objetivo desse estudo foi avaliar o ganho ósseo (GO) e a reabsorção do bloco enxertado (RB) após a utilização de enxertos homógenos para reconstrução óssea maxilar, bem como a relação destes com idade, tipo de tecido enxertado (TE) e espessura inicial de rebordo (EI). Utilizou-se 34 enxertos ósseos homógenos para reconstrução óssea em 24 pacientes, de modo que, na tomografia do pós-operatório de seis meses, foram analisadas as medidas de EI, GO, espessura óssea final (EF) e RB. Não se observou nenhuma reabsorção ou incorporação total dos blocos. O valor médio de GO foi 4,42 ± 1,43 mm e o de RB foi 2,04 ± 1,48 mm. Dos 34 blocos, somente dois apresentaram EF menor que 5 mm. Os valores médios de GO e RB não mostraram diferença estatística quando relacionados à idade, TE ou EI. Concluiu-se que os enxertos ósseos alógenos são uma alternativa de alta previsibilidade para aumento de volume ósseo maxilar, não parecendo ter relação com EI, TE ou idade


The human bone is a tissue that presents high mutability. When the resorption of alveolar bone shows itself in high levels, oral rehabilitation becomes difficult, mainly in cases of rehabilitations with dental implants. For this purpose, bone blocks started to be used in bone reconstruction for rehabilitation. These blocks can be classified as autograft, allograft, xenograft or alloplastic. Even though bone autograft is considered gold-standard, some side effects, like necessity of a secondary surgery and higher level of post-operative pain, led to an alternative found in bone allografts. The aim of this study was to evaluate the bone gain (GO) and the resorption of the grafted block (RB) after grafting allograft bone for maxillary reconstruction, as well as their relation with age, type of grafted tissue (TE) and initial bone width (EI). Thirty-four allograft bones were grafted in 24 patients’ maxilla and, in the six months post-operative tomography, the values of EI, GO, final width of alveolar bone (EF) and RB were analyzed. Neither total resorption nor total incorporation of the blocks was observed. The mean value for GO was 4,42 ± 1,43 mm and the one for RB was 2.04 ± 1.48 mm. Out of the 34 allografts, only 2 showed EF lower than 5 mm. No significant difference was observed when the mean values for GO and RB were related to age, TE or EI. In conclusion, allografts are a highly predicable alternative for enhancing alveolar bone volume, and did not present relation with TE, EI or age

Rehabilitation of edentulous areas has always been a challenge for dentists, especially on the posterior maxilla due to the pneumatization of the maxillary sinus. One of the complications during sinus lifting procedures is perforation of the membrane. The platelet-rich fibrin (PRF) is a complex of growth factors obtained from centrifuging the patient’s blood. It has the ability to accelerate the healing process and repair damages in the Schneiderian membrane, therefore, functioning as a filling material. ABSTRACT

Objetivo: Avaliar por meio da Tomografia Computadorizada singleslice, alterações nas estruturas do espaço mastigador, por disseminação loco-regional das neoplasias malignas originárias da loja tonsilar, trígono retromolar, seio maxilar e nasofaringe; correlacionar a presença de trismo com os achados tomográficos e dimensões do tumor. Material e métodos: foram selecionados prontuários de pacientes portadores de tumores malignos envolvendo as 4 regiões a serem estudadas, junto aos arquivos do Hospital Heliópolis. Foram excluídos os pacientes submetidos a terapêutica antineoplásica prévia ou com doenças inflamatórias ou infeciosas locais. Os prontuários foram revisados por um único examinador, onde foram coletadas informações relativas a idade, gênero, localização do tumor primário, tempo de evolução da doença, estadiamento do tumor e presença ou ausência de trismo. Após a administração do meio de contraste iodado, o protocolo de aquisição de imagens tomográficas foi de cortes axiais de 3mm de espessura, aquisições coronais foram feitas quando necessário. A análise das imagens foi feita por dois examinadores médicos, residentes do 3° de Radiologia médica, em momentos distintos, sem o conhecimento prévio das informações clínicas. Na avaliação do EM, os achados foram transcritos para uma planilha específica, considerando a presença ou ausência dos seguintes itens: simetria com o EM contralateral, obliteração do plano gorduroso, do trígono retromolar e do espaço faríngeo, edema e/ou atrofia dos músculos pterigoideos medial e lateral, destruição do ramo da mandíbula. Resultados:


O trismo esteve presente em 10 pacientes estudados. Na associação entre dimensão do tumor e trismo, observamos que 90% dos pacientes apresentam tamanho T4, enquanto essa porcentagem é de 43% nos pacientes sem trismo. Analisando-se em termos de razão de chances, verificou-se que pacientes com tumores T4 apresentam uma chance de trismo de 11,6 vezes maior que as demais dimensões (T0 a T3). As neoplasias da loja tonsilar e trígono retromolar perfizeram 95% das neoplasias avaliadas. Foram encontrados apenas 3 casos de neoplasia da nasofaringe, e em nenhum deles observamos a presença de trismo, e não foi encontrado nenhum caso de neoplasia de seio maxilar. Relacionando os achados tomográficos com o grupo de pacientes com trismo, observamos que 60% dos pacientes com trismo apresentaram edema e/ou atrofia dos músculos pterigoideos na TC, enquanto que essa porcentagem foi de 21,8% nos pacientes sem trismo. Em relação às dimensões do tumor e sua relação com os achados das imagens observamos associação apenas entre tumores T4 e detecção de edema e/ou atrofia dos músculos pterigoideos, neste item pacientes com edema e/ou atrofia dos músculos pterigoideos apresentam 5,4 vezes ais chance de apresentarem trismo. Conclusões: Observamos na TC as seguintes alterações no EM: obliteração do plano gorduroso em 69,2 % dos pacientes, assimetria em 27,7%, edema/atrofia em 26,2 % dos pacientes e destruição óssea em 20%. Os graus de concordância inter examinadores foram variáveis. Encontramos associação apenas entre pacientes com trismo e edema/atrofia dos músculos pterigoideos. Neste mesmo item foi observado associação com tumores de dimensão T4. Não encontramos associação do trismo com os demais itens avaliados na TC.


Objective: Evaluate by computed tomography (CT) singleslice changes in the structures of the masticator space, for loco-regional dissemination of malignancies originating in the tonsil, retromolar area, maxillary sinus and nasopharynx to correlate the CT findings with trismus and the tumor dimensions. Material and Methods: The medical records of patients with malignant tumors involving the four regions to be studied were selected from the archives of the Heliopolis Hospital. Patients submited previously to antineoplastic therapy or with local inflammatory or infectious diseases were excluded. The medical records were reviewed by a single examiner, where information was collected on age, gender, tumor location, disease evolution, tumor stage, and the presence or absence of trismus. After administration of iodinated contrast media, the protocol the image acquisition was axial slices 3mm, coronal acquisitions were made when necessary. The image analysis of the MS by two medical examiner residents in their 3rd year of Medical Radiology, at distinct moments , without prior knowledge of the clinical information. In the evaluation of the MS, the findings were transcribed to a specific worksheet, considering the presence or absence of the following items: symmetry with the contralateral MS, obliteration of the fat plane of the retromolar trigone and the pharyngeal space, edema, and/or the atrophy of the medial and lateral pterygoid muscles, and destruction of the mandibular ramus. Results: According to the adopted criteria for trismus in this study, the presence of trismus was observed in 10 patients. The association between tumor size and trismus shows that 90% of patients present size T4, while this percentage is 43% in patients without trismus. Analyzing in terms of odds ratios, the study showed that patients with T4 sized tumors had a chance of developing trismus 11.6 times greater than patients with T0 to T3 sized tumors.


The neoplasms of tonsillar crypts and retromolar trigone aggregated 95% of the neoplasms evaluated. Only 5% of the cases accounted for neoplasms of nasopharynx, in which none of them presented the existence of trismus. No cases of neoplasm of maxillary sinus was found. Correlating the CT findings with the group of patients with trismus, the observation was made that 60% of patients with trismus showed edema and/or pterygoid muscle atrophy on CT, whereas this percentage was 21.8% in patients without trismus. ABSTRACT

Although pulsatile tinnitus can be audible, objective demonstration of this heartbeat-synchronous sound has rarely been successful. We report a rare case of pulsatile tinnitus in a 44-yr-old female patient, which was induced by a large mastoid emissary vein (MEV) and objectively documented by Doppler sonography of the left posterior auricular region. The tinnitus was intermittent and the patient could adapt to the tinnitus without intervention on the mastoid emissary vein. These findings suggest that a single large MEV can cause pulsatile tinnitus in the absence of other vascular abnormalities, and imaging studies of the posterior fossa and Doppler ultrasonography can aid the diagnosis in such cases.

<p><b>OBJECTIVE</b>To investigate the potential utility of microangiography with synchrotron radiation to detect murine hepatocellular carcinoma (HCC) angiogenesis using an ex vivo model system.</p><p><b>METHODS</b>An HCC xenograft model was established by implanting HCCLM3 cells into male mice livers (n = 6). Twenty-eight days later, three of the mice were randomly selected for barium sulfate infusion into the liver and tumor via the inferior vena cava followed by ligation of the arteries, veins and common bile duct; the remaining three mice were left untreated and served as controls. All mice were sacrificed to collect livers for analysis using the BL13W beamline X-ray imager (Shanghai Synchrotron Radiation Facility, China). In addition, the tumor vasculature was evaluated by immunostaining of formalin-fixed tissues for CD31, CD34, and F8.</p><p><b>RESULTS</b>High resolution images of tumor angiogenesis were acquired and image analysis indicated that the normal blood vessels had been displaced by the fast growing tumors. Abundant and tortuous tumor angiogenesis in the tumor periphery area and sparse angiogenesis inside the tumor were also visualized clearly. These features were similar to the immunohistological results. The smallest tumor vessels visualized were approximately 20 mum in diameter.</p><p><b>CONCLUSION</b>Microangiography with synchrotron radiation using barium sulfate as contrast agent is a viable imaging strategy for tumor angiogenesis.</p>
